 Grants Pass Clinic we take our patient’s privacy and Protected Health Information (PHI) very seriously. It is our strict policy that all staff who access and share patient information as part of their job, do so on a ‘need to know’ basis only.  If it’s not needed for their work, they won’t be allowed to view it.  The Clinic has developed a thorough privacy training program starting with an employee’s first day and continuing with annual training and refreshers throughout the year. Our HIPAA Privacy Officer and HIPAA Committee perform regular audits of our electronic medical records system to assure that staff is only accessing patient information that is required to perform their work.

When there is a request to access a patient’s PHI by someone other than the patient, we have to determine if there is permission given for the information to be released.  Some circumstances require a power of attorney that include healthcare allowances for that person to access or view that patient’s information.  Sometimes there is guardian paperwork or foster care documentation that is required for our pediatric patients. 

Grants Pass Clinic also offers a patient portal application called Follow My Health (FMH). It allows patients to communicate confidentially with their care team, schedule appointments, and view and print some PHI. This includes clinical summaries of office visits, immunization records, lab results, medication lists, and other useful information.  If a patient wants to allow a designated person, such as spouse, grandparent, or partner, access to their FMH account they have the option of giving it through a proxy account. Please ask our Information Desk receptionist for the proxy form for either adult or pediatric patients.
